How to Register

Please come to the surgery and ask to register at the practice. Provided you are residing within our practice area and we are accepting new patients, you will be asked to complete a registration form. To register at this practice, you will need to produce the following documents to assist us with the registration process:

REF: Immigration Status- We require one of the following:

An up- to-date passport OR

  • If you are not normally resident in the U.K., a letter from the immigration department stating how long you have been permitted to stay in the country OR

  • If you are not normally resident in U.K. an I.D. card from the immigration department.

Proof of address- Any one of the following:

  • A bank statement

  • Recent utility bill

  • Driver license

OUR CATCHMENT AREA IS N8.

Appointments can be booked up to two weeks in advance.

If you are in the area for a short time we may only be able to see you as a temporary patient. Our staff can advise you. If you have come from overseas there is no automatic right to free treatment under the NHS. The rules are complicated and depend on your normal country of residence and the purpose of your stay. Please ask our staff for advice. Be aware that we may charge a fee for overseas residents in some cases.

In case of emergencies we will endeavor to accommodate patients according to their needs.

Please telephone our reception or call in. Please make a separate appointment for each family member. A standard appointment is ten minutes. Bear in mind that there is a limit as to how many problems the doctor can cover adequately in that time. If you do not have an appointment and need to be seen urgently we will try to fit you in but be prepared to wait.

It is most important you let us know as soon as possible if you are unable to keep your appointment so the staff can offer it to another patient. We record DNA’s where we are not informed in the medical record. It is now possible to order repeat medication on the internet (hornseyparksurgery@hotmail.co.uk).Additionally, www.patient.co.uk provides information on a variety of medical conditions and self help groups.
