Repeat Prescription Requests

Must be ordered in writing please using

• The request form issued with your medication (blank forms are available at the dispensary if you have lost it) This can then be placed in the box provided at reception, put through the letterbox or posted.

• By fax 020 8889 4715

Please write clearly and allow two working days for processing of your request. Prescriptions for dispensing from the chemist's shop will be sent directly, unless you ask to be able to collect it from the surgery.

Telephone requests are discouraged except for the housebound.

If you are on a number of regular medications, you may be able to save money on prescription charges by having a pre-payment certificate. Application forms for these and exemption certificates are available at the dispensary where the staff will be happy to advise you.
